NOI Notice of Intent/Timeline

Does anyone have a succinct POLICY on when you will or will not send a Notice (or Letter) of Intent (NOI)? We understand the use of a NOI, but the regs do not contain a policy on which circumstances may, shall, will, or might trigger a NOI being sent to a proposed displacee. How about some help! (OK, dig in!!)
